All patients will be asked for identification (ID cards, driver’s license or other form of identification) and an insurance card. Payment is required at the time of service. We accept cash, checks, Visa, Mastercard, Discover and American Express.
Please plan to arrive 15 minutes earlier than your scheduled appointment. It is also helpful to bring a list of questions for your doctor or other healthcare professional. Many times, patients can forget to ask questions when they are in our office. It’s important to us that all of your questions are addressed during your stay.
